Postpartum OCD

Postpartum OCD is a form of obsessive–compulsive disorder that emerges during pregnancy or after childbirth. It is characterized by intrusive, unwanted thoughts—often violent, sexual, or harm‑related—directed toward the baby, the self, or others. These thoughts are ego‑dystonic and deeply distressing; parents with postpartum OCD do not want to act on them. The fear of these thoughts often leads to compulsions such as avoidance of caregiving tasks, excessive checking, mental rituals, or seeking reassurance.

This subtype is frequently misunderstood because the intrusive thoughts can be alarming, but postpartum OCD is fundamentally an anxiety disorder—not a risk factor for harming a child. The distress comes from the mismatch between the parent’s values and the intrusive thoughts. ERP is the gold‑standard treatment, helping parents face feared situations without engaging in compulsions, while also reducing the power of intrusive thoughts over time.
